Cross-sectoral acute patient pathways
The acute healthcare system is rapidly evolving. Many initiatives are being launched, and new organizations are being created to meet the demand. From the patient's point of view, however, the healthcare system can seem increasingly fragmented.
An acute patient trajectory typically consists of multiple contacts across sectors, often including the primary sector, such as the GP or out-of-hours primary care services. When acute patient trajectories span multiple sectors, there is a risk of losing information that could otherwise support the patient's treatment.
Thus, there is a growing need to ensure and support the quality of emergency care across sectors. The research group 'Cross-sectoral acute patient pathways' therefore aims to conduct research on cross-sectoral patient pathways - with a particular emphasis on acute patients in and around general practice.
We wish to contribute with evidence-based evaluations of new initiatives and testing of tools that can contribute to more coherent pathways to the benefit of patients.
